Workplace accidents happen. These usually need to be documented
and preventative action taken. Often, the accident is recorded on paper –
either as a pro-forma or as a description – and frequently this occurs
some time after the incident has occurred. Not surprisingly then,
the quality of the steps taken to reduce the risk of similar accidents
recurring is sometimes reduced.
 
In addition, once the accident has been recorded and the steps taken
to reduce its recurrence, it is more than likely that the recorded details
will just be filed – never to see the light of day again.
 
But what if those details could be indexed and made available easily?
It is likely that Health and Safety managers would review the incidents
and take preventative measures to reduce the risk of similar accidents
occurring in the future.
 
The records could be captured in other formats – perhaps audio and
video – as well as the form-based and descriptive approaches. It is likely
that the record would be far more accurate; it could be taken almost
immediately after the accident had occurred. In addition, each incident
could be indexed, allowing cross-references to be built up over time.
Suddenly, past records would become useful again.
